This form must be completed for all refunds. It takes approximately 7 to 10 business days to process a refund. You must fill out all required fields before submitting this form!

If you would like to transfer to a different activity, please contact any park district center to assist you.

All refunds are subject to a $5.00 service fee. Transfers are not subject to a service fee.
